Owen Health Care is an approved provider with The Division of Developmental Disabilities (DDD) to provide support coordination services throughout the State of New Jersey. Owen Health Care is committed to enriching the lives of individuals through support and collaboration to ensure that support coordination services are delivered efficiently based on the individual’s needs.

Support coordination is one of the service models that provide services by identifying, advocating, developing, accessing, and coordinating supports on behalf of individuals that assist them in gaining access to needed programs and DDD services. Such programs can include needed medical, social, educational, and other types of good and/or services. We believe that support coordination also involves assisting individuals and/or their family’s access necessary supports on their own. All we do is assist and coordinate those efforts for and with them.

Who can receive services/enrollment:

  • Be a resident of the State of New Jersey
  • Be 21 years of age or older
  • Be DDD and Medicaid Eligible
  • Meet Functional Criteria
  • Complete the DDD Intake Application
  • Complete the NJ CAT/PCPT Assessments
  • Complete the NJISP
  • Complete and submit the Support Coordination Selection Form identifying Owen Health Care as your Support Coordination Agency.

If you entrust the coordination of your supports to Owen Health Care, you will be working with skilled and licensed professionals who will work as a team for and with you to identify not only the services that are needed, but the ones that are necessary for full and complete integration into the community. We will also bring to your attention through the DDD Database provider agencies who can better offer those services, where and how you can access those services, and aid you in making an educated and informed choice.

Our support coordinators are professionals that are dedicated, flexible, and provide person-centered planning to help improve overall quality of life. Our support coordinators are compassionate, and possess excellent communication, conflict resolution, and coordination skills. Our support coordinators are punctual, even at short notice. You will receive prompt and timely follow-up backed by solid teamwork and collaboration infused with deep knowledge and understanding of the DDD system, policies, and practices.

Our support coordinators use the person-centered approach, which targets:

  • Helping individuals/families build on their own capacities, strengths, and talents.
  • Identifying what brings joy to individuals and their families and finding ways to incorporate these things in their daily lives.
  • Observing and listening to focus in on what is working and what is not, and to help determine what individuals want more of and what things they want to change.
  • Helping people learn and practice how to stay healthy and safe.
  • Helping people understand risk and making choices that are meaningful to them.
  • Remaining optimistic during difficulties.
  • Being creative, flexible, and having fun.
  • Learning positive, growth-oriented lessons from mistakes.
  • Helping people develop valued and personally meaningful social roles. Supporting and encouraging relationships.
